phpWebForm is a dynamic form processing script written in php that is meant to be used in cases where an existing web site needs a form. It supports templates for mailing the form results, replying to the visitor, and displaying the thank-you page. It can also validate form input via JavaScript and insert the results into a database. Supports a variety of database types including MySQL, PostgreSQL, ODBC, etc using the PEAR DBI class.
